Thank you for supporting Orion Communities through a gift of stock or securities. You generosity helps advance our mission and strengthen the communities we serve.

1. Notify Orion Communities of you intent to transfer stock or other securities by
   contacting the Executive Director via email mqmirkil@orionhope.org or call 484-240-5725 or by completing the form below.
   Please be prepared to provide the following information:
   • Donor name, address, phone number, and email address
   • Name of the stock and number of shares being transferred
   • Gift designation or purpose
   • Anticipated transfer date
2. Provide your broker with the transfer instruction and account information: Wells Fargo DTC:0141; account # 8896-5299 Orion Communities Inc.
3. Once your broker confirms that the transfer has been completed, please notify us so we may confirm receipt of your gift and ensure it is properly designated.
4. After the assets have been received in Orion Communities’ account, no further
   actions is required. Orion will authorize Wells Fargo to sell the shares and will
   provide the donor with an acknowledgement letter reflecting the cash value of the gift for tax purposes.
